PUWER Inspections That Stand Up to Scrutiny

PUWER inspections made practical: plan checks, assign action, retain clear evidence and prove that work equipment is safe, suitable and controlled in use.
A failed guard, a missing pre-use check or an unreadable service record can turn ordinary work equipment into an immediate safety and enforcement issue. PUWER inspections give organisations a disciplined way to identify those failures early, control the risk and show exactly what was checked, by whom and what happened next.
For facilities and operations teams, the challenge is rarely knowing that equipment needs attention. It is maintaining a reliable process across sites, assets, shift patterns and contractors without letting inspection evidence disappear into inboxes, paper forms or separate maintenance systems.
What PUWER inspections are designed to prove
The Provision and Use of Work Equipment Regulations 1998 require employers to ensure work equipment is suitable, maintained and used safely. PUWER applies widely: powered machinery, hand tools, ladders, racking, cleaning equipment, lifting aids, kitchen equipment and workplace vehicles can all fall within scope where they are used by employees at work.
An inspection is not simply a tick-box exercise. It is a recorded examination of whether equipment has deteriorated, changed, been damaged or developed a defect that could create danger. The record should establish the equipment concerned, the date, the findings, the person completing the check and any action required.
The central operational question is straightforward: can the organisation demonstrate that the equipment was safe to use at the point it was put into service? If the answer depends on a supervisor's memory or a spreadsheet last updated months ago, the control is weak.
When inspections are required
PUWER does not impose one universal inspection interval for every asset. Frequency depends on the equipment, how it is used, its environment, manufacturer guidance, foreseeable failure modes and the consequences of a defect.
Inspections are required after installation where safety depends on installation conditions, and after assembly at a new site or location. They are also required where equipment is exposed to conditions that could lead to deterioration and create a significant risk. This can include intensive use, vibration, weather exposure, corrosive environments, impact damage or untrained use.
A sensible programme usually combines three layers of control. Operators may complete brief pre-use checks. Competent staff may carry out scheduled visual or functional inspections. Planned maintenance providers may complete more detailed servicing. These activities can overlap, but they should not be confused.
For example, a warehouse operative checking a pallet truck before a shift is not replacing a planned inspection of its brakes, wheels and hydraulics. Equally, an annual service does not remove the need to take equipment out of use when an operator identifies a fault on Tuesday morning.
Some equipment is subject to additional requirements. Lifting equipment may require thorough examination under LOLER, while pressure systems have their own inspection arrangements. A thorough examination is not the same as a PUWER inspection, even where both relate to the same equipment. Your asset register and inspection plan should make those distinctions clear.
Start with the equipment, not the form
The most reliable PUWER process begins with a complete, owned asset register. Before creating recurring checks, establish what equipment exists, where it is located, who is responsible for it and which controls apply.
For each asset or equipment group, record its identifier, site, department, make and model where useful, risk level, inspection frequency, maintenance requirement and responsible role. However, a single checklist for every item often creates false assurance. A ladder inspection needs different prompts from a bench grinder, extraction unit or powered door. The form should reflect real failure points, not merely prove that a form was completed.
A proportionate inspection prompt might ask whether guards are secure, emergency stops function, cables are undamaged, labels remain legible, safety devices are operational and the equipment is clean and stable. It should also give the inspector a clear route to mark a defect, attach a photograph and stop use where necessary.
Make defects trigger control, not administration
The inspection itself is only the first step. The quality of your response to a failed check is what protects people.
Where a defect presents serious or imminent danger, equipment should be isolated or clearly marked out of service immediately. Staff need authority to do this without waiting for approval. The record should capture the defect, its risk, any temporary control, photographs where relevant and the person notified.
Less urgent defects still need a controlled route into remedial work. Assign an owner, set a due date, track progress and require evidence before closure. If a contractor completes the repair, retain the service report, replacement details and any recommissioning check alongside the original defect.
This matters particularly across multiple sites. A central team may see that a check was failed, but without a connected workflow it can be difficult to tell whether the asset was isolated, whether a job was raised or whether it was returned to service prematurely. A closed action should mean the risk has been addressed, not that someone has acknowledged an email.

Over-inspecting low-risk equipment wastes time and encourages superficial completion. Under-inspecting higher-risk equipment leaves dangerous deterioration undetected. The right frequency is based on evidence, not habit.
Consider the equipment's use, age, load, location, known defects, servicing history and the likely consequences if it fails. A tool used occasionally in a controlled workshop may need a different regime from the same tool used daily on a construction-style maintenance round. Changes in use matter too. Equipment moved outdoors, shared between teams or used by less experienced staff may require more frequent checking.
Review frequencies after incidents, recurring defects, near misses, major repairs and changes to the workplace. This creates a useful feedback loop: the inspection programme becomes more precise as operational evidence builds.
Keep evidence ready for the real test
During an audit, investigation or enforcement visit, teams are rarely asked whether they have a policy alone. They are asked to show that controls operated in practice.
A defensible PUWER record should connect the asset to its risk assessment, inspection history, defects, maintenance activity, user training and supporting documents. It should also show overdue checks and unresolved actions clearly. A report that says 98 per cent of inspections were completed is less useful if the remaining 2 per cent includes high-risk equipment that was still in use.
This is where disconnected systems create unnecessary exposure. Inspection forms may sit in one application, maintenance records in another and training evidence in a shared drive. Reconstructing the story of one faulty machine then becomes a manual exercise at exactly the wrong moment.

Give people a process they can follow
PUWER duties depend on competent people. Inspectors need enough knowledge of the equipment and the defects they are expected to identify. Operators need to understand pre-use checks, safe operating limits and what to do when something is wrong. Managers need visibility of what is overdue, failed or awaiting repair.
Keep instructions specific and accessible at the point of work. A short, well-designed mobile check is more likely to be completed properly than a dense monthly form. But simplicity must not remove judgement: where a user is unsure, the process should make escalation easy.
