To help a maintained or academy primary or secondary school in England record how additional or alternative provision is being considered and coordinated for a severely absent pupil, based on the pupil’s assessed barriers and needs.

Use this plan where [pupil name] is absent from school for 50% or more of the time and may need more intensive support to overcome barriers to being in school. The school should work with relevant partners and select support according to the pupil’s assessed needs; no option is presumed necessary.

Record the attendance circumstances, the barriers identified, the pupil’s assessed needs and any information that may help partners understand what support could overcome those barriers.
