When to use this plan
Use this plan where [pupil name]’s SEND needs may not be adequately supported and this may be affecting attendance or continued participation. The plan should be proportionate to the pupil’s circumstances. It does not mean that an early EHC-plan review is required in every case.
Where unmet SEND needs may be contributing to reduced attendance or withdrawal from school, work closely with [pupil name] and the pupil’s parents to put appropriate support in place and keep it under regular review.
Pupil and family discussion
Discuss with [pupil name] and [parent or carer names] the support and adjustments that may be appropriate in response to the pupil’s SEND needs and the effect these needs may be having on attendance or continued participation.
Record the date of the discussion, the people involved, the concerns raised, the support and adjustments agreed or provided, and any relevant views from the pupil and parents: [discussion record location].
Support and adjustments
Put the appropriate support and adjustments in place for [pupil name]: [support and adjustments].
Record when the support or adjustments were provided and any information needed to understand their effect on attendance or continued participation: [support record location].
Regular review

Early EHC-plan review
If [pupil name] has an EHC plan and there are significant levels of unmet need, liaise with the local authority about whether an early review of the plan is needed. Local authority contact and liaison record: [local authority liaison record].
If the pupil does not have an EHC plan, or if significant unmet need is not identified, record the reason an early EHC-plan review was not pursued and continue to review the support and adjustments proportionately: [early review decision record].
