This procedure applies where [school name] has a powered gate, barrier, door or other work equipment and a dangerous condition or suspected safety defect is identified.
Immediate action
Anyone who identifies that a powered gate or other work equipment may be dangerous must report the condition immediately to [responsible role] and stop normal use.
Where work equipment or a powered gate is found to be dangerous, take it out of use immediately and keep it out of use until the safety concerns have been adequately addressed.
Isolation or supervised safe use
Switch off the equipment at [isolation location]. If the powered gate cannot be made safe immediately, use it only in a safe, supervised mode, such as under direct hold-to-run control, and only where that mode is suitable for the immediate situation.
Do not allow normal operation while the equipment remains unsafe. Restrict use until a competent person has taken the steps needed to make it safe.
Competent-person action
Arrange for a competent person to assess and make the equipment safe. This may include installing safety mechanisms or protective devices where they are needed.
Keep the equipment out of use while the competent person addresses the safety defect or other safety concern.
Return to service
Return the powered gate or other work equipment to normal use only after the competent person has made it safe and the safety concerns have been adequately addressed.
