To set out how a maintained school or academy in England identifies and addresses barriers to access for pupils and staff, records reasonable adjustment needs, and reviews progress.

Purpose and scope

This plan records actions to make the school buildings accessible to all. It covers identified accessibility barriers, assessment of particular needs, reasonable adjustment needs, responsible ownership, target actions, completion evidence and review arrangements.

Current position

Plan owner: [plan owner]. Date prepared: [plan date]. Planned review arrangement: [review arrangement]. The current position is recorded as: [current position].

Accessibility needs and assessment

For each relevant pupil or staff member, record whether the position is accessibility action identified, reasonable adjustment identified, no adjustment currently identified, or assessment not completed. Record the assessment date as [assessment date], the area considered as [assessed area], and the recorded position as [accessibility status].

Accessibility action register

Action reference [action reference]: the identified barrier is [identified barrier]. The action to make the building more accessible is [target action]. The person responsible is [responsible owner] and the target date is [target date]. The action status is [action status].

Where the action has been completed, record the completion date as [completion date] and describe the completion evidence as [completion evidence]. Where it has not been completed, record the current position and any revised target as [action update].

Reasonable adjustments

Where assessment of a pupil or staff member with particular needs identifies a need for a reasonable adjustment, record the adjustment as [reasonable adjustment], the person responsible for arranging it as [adjustment owner], and the date from which it is needed as [adjustment start date].

Where no adjustment is currently identified, record the basis for that position as [no adjustment record]. Where assessment has not been completed, record the outstanding assessment action as [outstanding assessment action].

Review and update

The plan is reviewed in accordance with [review arrangement]. At each review, record changes to identified barriers, assessment positions, reasonable adjustment needs, actions, owners, target dates and completion evidence in [review record location].

Next planned review: [next review date]. Review lead: [review lead].

Approval and communication

This plan is approved or adopted by [approving body] on [approval date]. The person responsible for communicating relevant arrangements to affected pupils, staff and other appropriate people is [communication owner].
