School Unsafe Gas Appliance Isolation and Recommissioning Procedure

This procedure applies when a gas appliance at [site name] is known or suspected to be unsafe. The appliance must not be used while that concern remains unresolved.
1. Identify and withdraw the appliance
Record the appliance identifier as [appliance id], its location as [appliance location], the concern identified, and the date and time of the report.
Stop use of the appliance immediately and prevent it from being returned to service while it is known or suspected to be unsafe. Record the withdrawal or isolation in the site’s gas safety record.
2. Arrange competent investigation
Notify [responsible person] and arrange investigation by a Gas Safe registered engineer or other competent person. Provide the appliance details, location, reported concern, and any relevant isolation or withdrawal information.
The competent person must investigate the reported condition and determine whether the appliance requires repair, replacement, or removal.
3. Repair, replace, or remove
Keep the appliance out of use while the competent investigation and any necessary corrective work are outstanding.
Arrange the repair or removal identified by the competent person. Retain the competent-person report and the relevant repair, replacement, or removal evidence with the appliance record.
4. Confirm safe status before return to use
Do not return the appliance to use until the competent person has confirmed its safe status following the investigation and any required repair, replacement, or removal.
Record the confirmation, the competent person’s name as [competent person name], the date of confirmation, and the supporting report or work evidence.
5. Recommissioning
Only after safe status has been confirmed may [responsible person] authorise the appliance’s return to use. Update the site record with the recommissioning decision and retain the supporting evidence.
