School Serious Child Incident Agency Notification Procedure

This procedure applies conditionally to a maintained or academy primary or secondary school in England when the school is the responsible employer and a child in its care suffers a serious accident, serious injury or death.
1. Recognise whether the procedure applies
Consider this procedure when a child in the school's care has suffered a serious accident, serious injury or death. Confirm that the school is the responsible employer for the relevant circumstances.
2. Notify the local child protection agencies
Notify [relevant local child protection agencies] of the serious accident, serious injury or death. Use the school's current notification route for those agencies: [local child protection agency notification route].
3. Record the notification
Record the incident, the notification made to the relevant local child protection agencies, the date and time of notification, the person making the notification, and any reference or confirmation provided: [incident notification record location].
4. Consider and act on advice
Consider any advice provided by the relevant local child protection agencies and act on that advice.
Record the advice received, the consideration given, the actions taken, and any further communication with the agencies at [incident advice and action record location].
5. Close the incident record
Keep the records of the serious accident, serious injury or death, the notification to the relevant local child protection agencies, and the advice received and acted on together in [serious child incident record location].
