School RIDDOR Report Submission Procedure

Use this procedure only where the incident is reportable under the applicable RIDDOR criteria. This procedure does not define the incidents or deadlines that trigger a legal duty.
1. Confirm that this procedure applies
Review the incident and confirm that it requires a RIDDOR notification under the applicable criteria before beginning the submission. If the incident is not reportable, do not use this procedure to submit a RIDDOR report.
If the reporting position is unclear, follow the school's escalation route through [riddor reporting escalation route] before submitting a notification.
2. Identify the reporter and incident record
The person preparing the notification must be the person designated under the school's reporting arrangements: [riddor reporter role].
Locate the underlying incident record at [site incident record location] and note its reference, [incident record reference], so that the submitted notification can be linked to the incident record.
3. Access the relevant online form
Open the relevant online RIDDOR form using [riddor forms access route]. Select the form that corresponds to the incident being reported.
Use the information in the underlying incident record to answer the questions presented by the form. Where the form requests information that is not available in the incident record, pause the submission and obtain the required information through [incident information escalation route].
4. Prepare and submit the notification
Check the answers entered in the online form against the underlying incident record before submitting the notification.
Submit the completed online form using the school's authorised reporting arrangements. Record the submission reference or other available confirmation with [incident record reference] at [site incident record location].
5. Amend a notification when necessary
If information relevant to a submitted notification changes, or if the notification needs correction, use the available process for amending the notification. The HSE reporting guidance includes amendments to notifications, including where a notified injury has ended in a fatality.
Update the linked incident record with the amendment details and retain the amended notification or confirmation with the original submission reference.
6. Close the submission record
Confirm that the RIDDOR submission is linked to the underlying incident record and that the record identifies the reporter, the submitted form or notification reference, and any amendment made.
