School RIDDOR Report Submission and Notification Amendment Procedure

This procedure applies conditionally to a maintained or academy primary or secondary school in England where an incident may be reportable under RIDDOR, or where an existing RIDDOR notification may need amendment. Actions must be based on the facts of the particular incident.
1. Identify the reporting responsibility
The [riddor responsible role] should establish whether the school is the employer or another person in charge of work premises for the incident under consideration.
RIDDOR requires employers and other people in charge of work premises to report and keep records of work-related accidents causing death, certain serious injuries, diagnosed cases of certain industrial diseases, and certain dangerous occurrences.
2. Assess whether the incident is reportable
Review the incident facts and classify the incident against the applicable RIDDOR reporting categories. Consider whether it involves a work-related accident causing death, a certain serious injury, a diagnosed case of a specified industrial disease, or a dangerous occurrence with the potential to cause harm.
Record the classification decision with the incident record reference [incident record reference]. If the facts do not establish that the incident is reportable, do not submit a notification on the basis of assumption alone.
3. Prepare and submit the notification
Where the incident is reportable under RIDDOR, prepare the notification using the relevant RIDDOR reporting route and the facts available for the particular incident.
Before submission, check that the notification information is consistent with the incident record and that the reporting decision is recorded. Submit the notification through the applicable RIDDOR process.
4. Retain submission evidence
Retain the submitted notification record and the available submission confirmation or other evidence of reporting in [submission evidence location].
Link the retained notification evidence to the incident record reference [incident record reference] so that the classification decision, submitted notification, and reporting confirmation can be located together.
5. Review a notification when facts change
Keep an administrative process for reviewing and amending a RIDDOR notification when relevant facts change after submission.
Review subsequent incident or medical information that may change the report. This includes a case where an injury that has already been notified subsequently results in a fatality.
Record the changed fact, the amendment decision, and the relationship between the original notification and any amended notification or confirmation. Retain these records with the incident record and the original submission evidence.
6. Close the administrative record
Ensure that the incident record contains the reporting classification, the submitted notification record where applicable, the submission evidence, and any amendment decision or amended notification evidence.
