RIDDOR Incident Assessment and Reporting Procedure

This procedure applies where a work-related accident or incident at the school may meet the applicable RIDDOR reporting criteria. It is intended for use by [responsible person role] and others who identify, receive or manage information about such an incident.
Immediate incident record
Record the incident promptly using [incident recording system]. Include the date, time, location, people involved, a factual description of what happened, immediate actions taken and the contact details of people who can provide further information.
RIDDOR assessment
Assess whether the incident is within the scope of RIDDOR and whether it meets the applicable reporting criteria. Use the current RIDDOR requirements when checking the detailed thresholds; do not rely on an outdated summary or an assumption based only on the apparent seriousness of the incident.
The assessment decision must be recorded as either reportable, not reportable or awaiting further information. Record the reasons for the decision, the person making it and the date of the decision.
Responsibility and escalation
The person receiving the initial notification must send the incident information to [responsible person role] without avoidable delay. Where the available information is incomplete or the reporting status is uncertain, escalate the matter to [escalation role] and record the escalation.
The school must identify the person responsible for making any required report and record that allocation against the incident.
Submitting a report
Where the assessment concludes that the incident meets the applicable RIDDOR reporting criteria, the responsible person must submit the report through [riddor reporting route] using the current reporting process and retain the submission reference and submission date.
If the assessment concludes that the incident is not reportable, retain the assessment decision and the information on which it was based in [incident recording system].
Record retention and review
Keep the incident details, assessment decision, escalation record, responsible-person allocation and any submitted report information together in [incident recording system]. Review the procedure when the applicable RIDDOR requirements or the school’s reporting arrangements change.
