Enter the date on which the incident occurred.

Use the school's usual 12-hour or 24-hour time format.

Record the specific location, such as playground, science laboratory or classroom 12.

Select the role or roles of the person or people who attended the incident.

Record the person's name and, where relevant, their pupil, staff or visitor status in accordance with the school's information-handling arrangements.

Select all categories that describe the injury or illness.

Describe what happened and the apparent injury or illness. Record observable facts and avoid assumptions.

Select all first-aid measures provided and add details in the description field.

Record the treatment, advice, monitoring or other assistance provided.

Select the outcome immediately after first aid was provided.

Include relevant details such as who collected the person, where they went, or any advice given.

Record the full name and role of the person completing or responsible for this record.

Complete this field only where further action, follow-up, referral or a related notification was identified.

Indicate whether this entry is also recorded in the statutory accident book or another incident record.
