This procedure applies conditionally where the school is a registered early years provider for the relevant provision.
When to use this procedure
Use this procedure when a serious accident, illness or injury happens to a child in the school’s registered early years provision while the child is in the school’s care, or when a child in that care dies.
Immediate recording
Record the incident, the date it occurred, the child affected, and the action taken. Record the date and time at which the notification is made and retain the notification record and submission confirmation.
Notification
Notify Ofsted or the registered childminder agency with which the provider is registered. Use the applicable notification route for [registered provider notification route].
Make the notification as soon as reasonably practicable and, in any event, within 14 days of the incident occurring.
Completion of the incident record
Ensure that the incident record includes the action taken, the date of the incident, the date of notification, the recipient, and the submission confirmation. Store the record and confirmation in [incident record location].
Responsibility
The person responsible for the registered early years provision should ensure that a notification is made within the required period and that the incident record is retained.
