This procedure applies to a maintained or academy primary or secondary school in England when the school is acting as the employer responsible for a child in its care and the child suffers a serious accident or injury, or dies.
1. Identify whether the procedure applies
When an incident involving a child in the school's care is reported or identified, determine whether it involves a serious accident, serious injury or the death of the child. If it does, continue with this procedure.
2. Notify the relevant local child protection agencies
Notify [relevant local child protection agencies] of the serious accident, serious injury or death involving the child. Record the identity of the agencies notified and the notification made.
3. Record the incident and notification
Create or update the school's record for the incident, including the serious accident, serious injury or death, the notification to the relevant local child protection agencies and any advice received.
4. Act on advice received
Consider advice provided by the relevant local child protection agencies and act on that advice. Record the advice received, the action taken and any further action identified.
