RIDDOR Notification Amendment Procedure

This procedure applies to a maintained or academy primary or secondary school in England that has submitted a RIDDOR notification and later receives information that may change the notification.
1. Identify information that may affect the notification
When information is received that may change a submitted RIDDOR notification, the person responsible for RIDDOR records at [school name] should link the new information to the original notification and begin an amendment review.
Information requiring review may include subsequent incident or medical information. A notified injury that subsequently results in a fatality must be included in the review.
2. Review the submitted notification
Compare the new information with the submitted notification and identify which reported facts may have changed. Record the review date, the notification reference, the information considered, and the person completing the review.
The review should determine whether the changed information affects the notification and whether an amendment is needed under the applicable HSE process.
3. Record the amendment decision
Record one of the following decisions against the original notification: amendment required; amendment not required; or decision pending further information.
For an amendment-required decision, record the facts that have changed and the reason the notification requires amendment. For an amendment-not-required decision, record why the reviewed information does not change the notification. For a decision pending further information, record what information is awaited and the next action.
4. Submit and retain the amended notification
Where an amendment is required, use the applicable HSE process for amending the notification and ensure that the amended notification is submitted by [responsible person name].
Retain the amended notification or submission confirmation with the original notification, the information that prompted the review, and the recorded amendment decision. If the applicable process does not produce a separate confirmation, record the submission details available to the school.
5. Fatality following a notified injury
If a notified injury subsequently results in a fatality, review the original notification specifically for amendment and follow the applicable HSE process for amending the notification. Record the review, decision, submission or confirmation, and retained records in the same notification file.
6. Record location
Store the completed amendment record and related notification documents in [riddor record location], linked to the original notification reference.
